Secondary Transition Services
According to IDEA, Secondary Transition Services are:
A coordinated set of activities designed to facilitate access and participation in the following post-school domains:
- Post-secondary education, vocational education, integrated employment, continuing and adult education, independent living, and/or community participation.
Transition services and activities are outlined within each IEP’s Transition Plan. These services are based on individual strengths, interests, access needs, and preferences related to the following.
- Instruction;
- Related services;
- Community experiences;
- Development of post-school living outcomes;
- Functional evaluation in the areas of daily living and vocational capacities.
