In a lifetime of transitions, the change from high school to adulthood is a big one. Planning for what comes after high school graduation is a challenging responsibility for all students. Students with disabilities may face additional barriers in emerging adulthood, as they work with their families (and school teams) to answer the question - What's next? With so much to plan for (employment, community participation, independent living, education, etc.), this can be a daunting task. Fortunately, early and ongoing transition planning can help students develop greater self-determination, which will then support their overall career and/or adult living goals. Accessing family/community-based resources provide essential information and tools which can give families a leg up.
