Welcome to our webpage for Secondary Transition! This webpage will house valuable information and resources that will help you navigate transition services for secondary students in CHCCS.
We prioritize partnering with students, families, and community support networks. Together, we create a shared vision for guiding postsecondary planning for Employment, Education, Training, and Community Living. In alignment with our district’s mission, we prioritize the highest quality and most engaging learning experiences for our students. We believe in the power of the home/school/community partnerships to ensure diverse perspectives. We prioritize student-centered planning so that students graduate believing in their full potential to live happy, inspired, and self-determined lives as adults.
The WHAT and WHY of Secondary Transition Services:
In accordance with North Carolina Policies Governing Services for Children with Disabilities (amended August 2020) and the Individuals with Disabilities Education Act (IDEA), CHCCS provides Special Education and Related Services for students with disabilities, ages 3 through 21. Students identified with a disability and found eligible for Special Education are offered services, as appropriate, to meet their unique needs as specified in their Individual Education Program (IEP). According to IDEA, all students with IEPs age 16 and up (or younger if deemed appropriate) will have the following:
- Measurable postsecondary goals based upon age-appropriate transition assessments related to post-secondary education, employment, and, where appropriate, independent living skills
- A Transition Plan which outlines the Transition Services (including courses of study) needed to assist the student in reaching those goals.
- **As a supplement to IDEA Transition Services, CHCCS provides Pre-Employment Transition Services (Pre-ETS) through the Transition Ready Agency Collaborative (TRAC). More about Pre-ETS and TRAC.
CHCCS provides Transition planning and interventions for students with disabilities to support their changing roles as 21st century learners, workers, and community members.
